The LPCSO no longer requires a copy of the newly issued ID card or driver’s license.

NOTE: Sex offenders who are new to the state of Arizona will need to have their birth certificate, a government issued photo ID, Social Security card. Offenders who do not have an original or certified copy of their birth certificate will need to start the process of obtaining one immediately. Check with the birth state for their requirements.

5. Risk Classification Level: Sex offenders moving to Arizona are required to be assessed to obtain a Risk Classification Level. This will be completed by using a National Criminal History check, documentation from other agencies involved in the offender’s arrest, as well as detention records and community supervision. Any documentation that can be provided may aid in the process.

Offenders who have documentation that they successfully completed any sex offender treatment programs, or drug/alcohol programs, will need to bring those documents to their appointment. Only documented completion of these programs will be considered for the assessment.
